Required forms

Every family signs these before a spot is confirmed — liability, photo release, allergy and studio policies. These can't be skipped.

Liability Waiver & ReleaseRequired

I acknowledge that participation in Munch & Co. culinary classes involves the use of kitchen tools, heat sources, and food ingredients. I voluntarily assume all risks of participation and release Munch & Co., its owners, instructors, and volunteers from any liability for injury, illness, or loss of property, except in cases of gross negligence. I confirm my child is physically able to participate and that I have disclosed all relevant medical information.

Photo & Media ReleaseRequired

I grant Munch & Co. permission to photograph and record my child during classes and events, and to use those images and recordings for marketing, social media, website, and print materials without compensation. I understand I may withdraw this permission in writing at any time.

Allergy & Medical DisclosureRequired

I certify that I have disclosed all food allergies, dietary restrictions, sensitivities, and medical conditions for my child on this registration. I understand classes handle common allergens including wheat, dairy, eggs, nuts, soy, and shellfish, and that Munch & Co. cannot guarantee an allergen-free environment. I authorize instructors to seek emergency medical care if needed.

Pickup & Supervision AuthorizationRequired

I understand that only I or an adult I have authorized in writing may pick up my child. Mini Chef students may NOT leave the premises at any time during class and must be signed out by an authorized adult. Late pickup beyond 15 minutes may incur a fee.

Studio Policies AcknowledgmentRequired

I have read and agree to Munch & Co. studio policies: (1) Mini Chef students may not leave the premises during class. (2) Parking is limited — I will not block neighbors' driveways, mailboxes, or private parking. (3) Munch & Co. is cashless in person; all in-person purchases are by card. (4) Closed-toe shoes and tied-back hair are required in the kitchen. (5) Tuition and class fees follow the posted refund and make-up policy.

Tuition & payment

Due today$149.00

Includes the first month of tuition plus the $149 annual student investment per student — apron, knife kit, onboarding materials, portfolio and academy setup. Drop-in classes are $85 each.

We are a direct Step Up For Students provider — search MUNCH AND CO in the EMA marketplace. In person we are fully cashless; adult pop-ups and merch are card only.

Studio policies

Mini Chefs may not leave the premises at any time during class — a parent or guardian must sign them out.
Parking is limited. Please do not block neighbors' driveways or entrances.
All payments are non-refundable — classes, pop-ups, events, kits, and fees.
A booking may be rescheduled up to 2 times, and only if space permits (the class or pop-up you move into must not already be full).
Pop-up and class spots must be confirmed when we text and email you 24 hours before — unconfirmed spots may be released.
